KEY ACCOUNTABILITES:

    • Collaborate closely with nurse practitioners, nurses, social workers, dietitians, and other health professionals to ensure coordinated, seamless care.
    • Participate in case reviews, clinical team meetings, and quality improvement initiatives.
    • Provide consultation and mentorship to clinical staff and learners as required.
    • Maintain accurate, timely, and confidential medical documentation in accordance with professional and organizational standards.
    • Engage in health promotion and education initiatives, including community outreach and patient self-management support.
    • Contribute to the development, delivery, and evaluation of clinical programs such as prenatal care, chronic disease prevention, cardiovascular health, and palliative care.
    • Support “seamless care” by ensuring appropriate referrals and coordination among specialists, hospitals, and community partners.
    • Be accessible during scheduled hours and provide on-call coverage as required.

REQUIREMENTS:

    • Doctor of Medicine (MD) and registration (or eligibility for registration) with the College of Physicians and Surgeons of Ontario (CPSO).
    • Certification in Family Medicine from the College of Family Physicians of Canada (CFPC) preferred.
    • Current CMPA membership in good standing.
    • Demonstrated commitment to community-based primary care and interdisciplinary collaboration.
    • Excellent communication, problem-solving, and organizational skills.
    • Commitment to health equity, cultural safety, and patient-centered practice.
    • Experience in community health, chronic disease management, or health promotion considered an asset.

COMPENSATION:

The wage for this position, provided in accordance with Ontario Health (East) funding for this role, is $170.24 per hour.
